The Health Resources and Services Administration (HRSA) announced it is awarding up to $200,000 a year to rural primary care providers to improve patients’ health and reduce hospital visits related to chronic disease. Individuals and health centers or clinics in rural areas are eligible to apply.

Applications must be submitted before March 4.
