Abstract Submission Form

In some cases, we have multiple abstracts submitted on the same topic. When this occurs, we might look at creating a panel discussion session with multiple speakers. For panel sessions, the honorarium is split between the speakers, with a minimum of $100 for each presenter.

If your topic overlaps with another speaker, would you be willing to participate in a panel presentation instead of a session of your own?

If you are submitting an abstract for a concurrent or workshop session and are not selected, are you interested in presenting your topic in a poster presentation?

Future speaker forms will be in Microsoft Word format. If you DO NOT have access to Microsoft Word, please mark below so that we can accommodate
